• When insured by this plan, you will be eligible for benefits when you require:
  
  
  • substantial assistance from another person to perform 2 or more activities of Daily Living:
    
    

    Eating    Bathing    Dressing

    Toileting    Continence    Transferring Position

    
    
  • substantial supervision required due to severe cognitive impairment... an inability to think, reason, or remember.
  
  
• Once you are eligible for benefits your Patient Advocate will assess your needs and develop a Plan of Care.
  
  
• Benefits will begin after you've received 30 days of Care.
  
  
• You will be paid eligible expenses based on your chosen Plan, A, B or C.
